Ledgewood Rehabilitation is a short term rehabilitation unit within Branford Hills Health Care Center. Ledgewood is a separate and dedicated unit designed for those individuals who no longer require acute hospital care but are unable to return home without additional care and services.

 

     Ledgewood provides the necessary rehab and medical programs to help patients reach their maximum potential and return home to their families and communities. We also have an extensive outpatient physical and occupational therapy department to complete the continuum of care for our patients. In order to provide the highest quality of care, our focus has always been on the individual patient. Our compassionate and professional staff uses an interdisciplinary team approach to assess, develop, implement, and review each patient in an effort to reach their maximum potential.

 

     Our clinical programs are designed for those patients with medical complications that inhibit their recuperation. Our nursing staff is trained and experienced to treat patients who have complex medical conditions or require special interventions.

We also address the needs of those residents who require rehabilitation services and programs resulting form Post Surgical Conditions, Cardiac Recovery, Orthopedic Recovery, and Stroke Neurological Rehabilitation.

 

     Our in-house staff of therapists design individual therapy programs that focus on each patient’s functional ability and rehab needs. We emphasize and promote the outcomes you desire by teaching strengthening, independence and activities of daily living
